Making a Referral

signAll referrals to Pastoral Cymru are dealt with by our Referrals Manager, who provides a single point of contact between local services and our multi-disciplinary team. All new referrals are considered and discussed by the team, who will initially determine whether the referral meets the admission criteria.

We will aim to undertake an assessment of appropriate referrals within 5 working days (unless otherwise prioritised).

Following assessments the multi-disciplinary team will meet to consider the outcome of the assessment. Immediate contact will be made following this and the outcome communicated to the referring team.

Treatment framework

buzzerOur treatment process following admission covers the following aspects:

  • Behavioural stabilisation
  • Baseline assessment
  • Intervention and risk reduction; and
  • Rehabilitation and social integration

This process will formulate a treatment plan that will aim to meet the service users' individual identified needs (Specific Individual Treatment). The treatment plan will identify the interventions, treatment goals and risk management measures required, which will be delivered in accordance with the Care Programme Approach.

Collaboration

Pastoral Cymru expect continued involvement and engagement with local services at every stage of the care pathway. This is particularly important in relation to discharge planning and after-care.

Local services will be invited to attend Care Programme Approach and other key meetings and will be encouraged to participate in discussions relating to the ongoing care and treatment of their patients.
